The Change of Payment Request allows you to update the current debit information we have saved on file for your swimmer(s) monthly automatic deduction as well as take care of any declined automatic debit transaction.

The Payment/Informed Consent & Waiver form is required for every student and must be submitted prior to your first lesson.

The Cancellation Request allows you to cancel your lessons and removes you from our swim schedule therefore stopping the monthly automatic deductions.
